ACL SURGERY 2.0 — INTERNAL BRACES, SCAFFOLDS, AND SMARTER GRAFTS

By Dr. Samir Pilankar, Robotic Knee Surgeon

Anterior Cruciate Ligament (ACL) tear is the commonest sports injury encountered. Arthroscopic ACL reconstruction has been the accepted standard of treatment for the past two decades with excellent results. Over the last few years ACL surgery has entered an exciting new phase with techniques that focus on preserving tissue, enhancing graft strength, and promoting natural healing. From remnant preservation and internal bracing to artificial and scaffold grafts, along with refined allografts, these advances promise better outcomes and quicker recovery for patients. Together, these cutting-edge approaches mark a shift from traditional reconstruction toward biologically friendly, patient-specific solutions that may redefine the future of knee ligament surgery.

Dr. Samir Pilankar is a reputed Arthroscopy & joint replacement surgeon with 28 years of experience & is rated among the best in Mumbai. He is a specialist in mini-incision technique in robotic joint replacement surgery and arthroscopy resulting in faster recovery. He is one of the few surgeons in India performing revision Arthroscopy & Arthroplasty surgeries with excellent results. After extensive training in renowned hospitals in Switzerland, Spain & Germany, he has successfully performed over 5000 Arthroscopy & Joint replacement surgeries. He was awarded with the prestigious Abdul Kalam Award, Times Icon award 2022 and featured in India Today’s top orthopaedic surgeons 2023. At the recently concluded SICOT conference in Kyoto, Japan, his presentation on the Pilankar technique in hip replacement surgery drew enthusiastic applause from the audience.  In this article, he has shone light on the recent advances in ACL surgery in question-and-answer format.

Q: What’s changed recently in ACL surgery?

A: Surgeons now individualize reconstruction/repair using five key innovations: remnant-preserving techniques, suture-tape “internal brace” augmentation, modern artificial (synthetic) ligaments, biologic scaffold–assisted repair, and improved allograft selection. These aim to speed healing, protect the graft, and better restore knee stability and proprioception.

Q: What is remnant preservation and why consider it?

A: Instead of fully removing torn ACL fibers, surgeons keep useful native tissue (especially on the tibial side) and integrate it with the new graft. Preserving this “stump” may enhance revascularization, proprioception, and early stability, with meta-analyses showing small but meaningful gains in functional scores and laxity. Ideal candidates have a substantial, vascular remnant without cyclops risk.

Q: How does an “internal brace” help?

A: A high-strength suture tape is added alongside the graft to share load during early rehab, increasing construct stiffness and ultimate load in bench and early clinical studies. It’s most useful in high-risk athletes or revisions, though evidence is still evolving and some reviews call for more robust long-term data.

Q: Are artificial (synthetic) ACL grafts back?

A: Modern synthetics such as LARS (polyethylene terephthalate fibers) have re-emerged with better designs and selected indications (e.g., older or lower-demand patients seeking faster recovery). Early mid-term series report acceptable stability, but long-term durability and broad use remain debated; many centers still prefer biological grafts for young pivoting athletes.

Q: What is scaffold-assisted ACL repair (e.g., BEAR)?

A: Bridge-Enhanced ACL Repair (BEAR) uses a resorbable collagen scaffold between torn ends, saturated with the patient’s blood, to stimulate native-ligament healing rather than replacing it. The device received FDA De Novo clearance in 2020, and mid-term data suggest outcomes comparable to hamstring ACL reconstruction in selected tears; indications have expanded as evidence accrues.

Q: Where do allografts fit today?

A: Allografts (donor tissue) avoid harvest morbidity and can shorten surgery, but multiple studies show higher failure rates in young, high-demand patients. They are reasonable for older, lower-demand individuals or complex multiligament injuries; graft processing (e.g., irradiation) and patient selection critically influence outcomes.

Q: Bottom line for choosing among these options?

A: Match technique to patient and tear pattern: consider remnant-preserving ACLR when good tissue remains; add an internal brace for high-risk loads; consider BEAR for repair-amenable complete tears; reserve synthetics and allografts for specific indications. Shared decision-making, meticulous tunnel placement, and criterion-based rehab still drive success.
