According to him, many patients ignore early warning signs. Occasional soreness after a long walk or workout may settle with rest, ice, and basic care. However, persistent symptoms are often the body’s way of signaling an underlying structural issue — such as cartilage wear, meniscus injury, ligament strain, early arthritis, or malalignment.

“The problem isn’t the temporary relief,” he explains. “Painkillers and supports can mask symptoms. But if the pain returns once medication stops, it suggests the root cause hasn’t been addressed.”

Dr. Pilankar emphasizes that the 3-week rule is not meant to create panic, but clarity. Knee conditions detected early can often be managed with physiotherapy, targeted injections, alignment correction, or minimally invasive arthroscopy. Delayed presentation, on the other hand, may lead to worsening cartilage damage, deformity progression, or advanced arthritis requiring joint replacement.

Red flags that should not be ignored include:

Meniscus tears are among the most commonly overlooked knee injuries. What begins as intermittent pain, occasional locking, or discomfort while squatting is often dismissed as a “minor strain.” However, untreated meniscal tears can propagate, especially in the avascular zones where healing potential is limited. Over time, a repairable tear may become irreparable, leaving partial meniscectomy as the only option. Loss of meniscal tissue compromises load distribution, accelerates cartilage wear, and increases the long-term risk of early osteoarthritis.

Similarly, an undiagnosed anterior cruciate ligament (ACL) tear can have progressive consequences. Patients may adapt to instability initially, but recurrent episodes of the knee “giving way” generate shear forces within the joint. These instability events frequently lead to secondary damage — including meniscal tears and focal cartilage lesions. Delayed diagnosis reduces the likelihood of preserving internal structures and may convert a straightforward ligament reconstruction into a more complex procedure with additional cartilage or meniscal repair requirements.

“In orthopedics, time matters,” Dr. Pilankar adds. “Three weeks is a reasonable window to observe. Beyond that, get clarity. Your knees carry you for life — don’t wait until damage becomes irreversible.”
